Pre-diabetes Brief Intervention Support

Our practitioners are trained in supporting individuals who are at risk of developing Type 2 Diabetes to reduce their risk. We can offer short sessions that seek to motivate and support people to plan changes in their lifestyle to reduce their risk.

The intervention aims to provide patients with evidence-based information around diabetes prevention as well as providing behavioural support to make changes to reduce their risk.

Length of programme

The intervention typically lasts around 12 weeks, but may last longer depending on individual needs.

Frequency of appointments

There are 6 structured sessions with topics around diabetes prevention. The appointments are typically delivered fortnightly for 12 weeks with a follow up after 6 months of the intervention.

Referral criteria

Adult 18+

Diagnosed with “pre-diabetes” through blood test

Or High/Very High risk through Diabetes UK “Know Your Risk” Score
